Privacy (personal information protection)

しば胃腸こうもんクリニック (“the clinic”) protects personal information under the following policy, and ensures every staff member understands and practices it.

• Management: we keep your personal information accurate and up to date, and protect it against unauthorized access, loss, damage, alteration and leakage through security systems, managed procedures and staff training.
• Use: information you entrust to us is used to contact you, provide our services, and answer your questions — by email or by post.
• No third-party disclosure: we do not disclose your information to third parties except with your consent, to contractors engaged to deliver a service you requested, or where disclosure is required by law.
• Your rights: if you wish to review, correct or delete your personal information, we will respond after verifying your identity.
• Compliance: we comply with applicable Japanese law and review this policy for continuous improvement.

Risk management

The director completed the medical association's patient-safety training program (February 2014) and passes its monthly examinations. The clinic runs a no-blame incident-reporting system: events and near-misses are shared with all staff so problems can be addressed openly and creatively rather than hidden. We take this lesson from the 1984 Space Shuttle disaster recovery — never blame the individual who reports.

Infection control
